We report a case of twenty year-old man with a six weeks history of cough and shortness of breath on exertion. The chest radiograph demonstrated a bilateral symmetrical micro nodular pattern. High resolution computed tomography revealed bilateral diffuse fine nodular shadowing involving mid zones with sparing of apices.
