Hypercarbia and respiratory acidosis continued to increase despite maximal ventilation, bronchodilator therapy, sedation, and paralysis. Mistaken use of non-partitioned ventilator circuit was the cause of the hypercarbia. The ventilator{\textquoteright}s self-test function failed to detect the error.
