Zusammenfassung
Ziel
Im Erwachsenenalter werden zur Intubation standardmäßig endotracheale Tuben mit Blockungsmanschette (Cuff) eingesetzt. Hierbei steigt der Cuffdruck im Verlauf einer Narkose mit Lachgas (N2O) an. In der vorliegenden Arbeit sollten das intraoperative Cuffdruckverhalten und die Inzidenz postoperativer Beschwerden bei dem Einsatz eines neuen automatischen Cuffdruckreglers überprüft werden.
Methodik
In einer prospektiven, randomisierten, offenen Studie wurde intraoperativ der Cuffdruck mit dem Cuff Pressure Control® der Firma Tracoe bei 40 Patienten gemessen und der voreingestellte Zielwert von 25,5 cmH2O mit einem geeichten Handmanometer überprüft. Die Daten wurden mit denen einer gleichgroßen Kontrollgruppe verglichen, bei der die Cuffdrücke auf konventionelle Art gemessen wurden und erst beim Überschreiten eines Druckwertes von 40 cmH2O abgelassen wurde. Eine Befragung der Patienten fand 2 h und 24 h postoperativ statt. Hierbei wurden Beschwerden, wie Halsschmerzen, Heiserkeit, Schluckbeschwerden und Hustenreiz erfasst.
Ergebnisse
Der Cuffdruckregler hielt den Cuffdruck in einem Bereich von ±2 cmH2O konstant auf dem voreingestellten Wert. In der Kontrollgruppe stieg der Cuffdruck einem Sättigungsverhalten folgend an. Die Inzidenz der postoperativen Beschwerden war in beiden Gruppen vergleichbar.
Schlussfolgerung
Die vorliegende Untersuchung zeigt, dass der hier eingesetzte Cuffdruckregler zum intraoperativen Cuffdruckmonitoring im untersuchten Druckbereich bei Erwachsenen geeignet ist.
Abstract
Objectives
In adults the use of cuffed endotracheal tubes is the standard technique. Nitrous oxide increases the cuff pressure secondary to diffusion through the cuff membrane. The aim of the study was to verify a newly designed cuff pressure regulating device and comparison of postoperative complications.
Methods
In a prospective, randomized, open trial the cuff pressure and the incidence of postoperative complications (e.g., hoarseness, coughing and pain while swallowing) were measured using the newly designed automatic pressure monitoring and regulating device Cuff Pressure Control® (Tracoe) and a conventional handheld manometer. A total of 40 patients were assigned to the automatic group with a goal of 25.5 cmH2O and 40 patients to the conventional group where the pressures were kept between 20 cmH20 and 40 cmH20 with intermittent measurements and manual pressure release.
Results
The automatic device reliably maintained the pressure at the chosen constant level within ±2 cmH2O. In the control group increases in cuff pressure to 40 cmH2O were common. The incidence of postoperative complications in both groups was not significantly different.
Conclusions
In conclusion our data demonstrate that the automatic cuff pressure and regulation device was useful and reliable in an adult population of intubated patients in the studied pressure range.
