Pulmonary alveolar proteinosis presenting with pneumothorax

A 32-year-old man who presented with pneumothorax was found at autopsy to have pulmonary alveolar proteinosis. Chest x-ray showed a honeycomb appearance and rupture of a subpleural bulla is suggested as the probable cause. Variations in radiological patterns and in their distribution are discussed.
