A single dose of nebulized budesonide decreases exhaled nitric oxide in children with acute asthma

Abstract

This study was conducted to investigate whether a single dose of nebulized budesonide effectively decreased airway inflammation as demonstrated by exhaled nitric oxide (eNO) levels. A single dose of nebulized budesonide, but not nebulized terbutaline, rapidly decreased eNO levels in 6 hours. The decrease in eNO levels induced by nebulized budesonide was correlated to an increase in peak expiratory flow rate. (J Pediatr 2001;139:433-7)

Patients

A total of 30 children with asthma between the ages of 6 and 17 years attending our emergency department were enrolled for the study. Informed consent was obtained from the parents in every case. Demographic data and initial disease status in the control and experimental groups

There were no significant differences in the demographic data between control and experimental groups with respect to age (10.25 ± 2.80 years in control group; 9.08 ± 2.19 years in budesonide group), sex, eNO level, and blood pressure. The patients in each group had similar degrees of illness at entry as reflected by mean PIS, mean respiratory rate, and mean PEFR. There were no significant changes in blood pressure before and after treatment (Table).

Discussion

Evidence has indicated that a single intramuscular injection of methylprednisolone results in a decrease in the hospitalization rate for children with acute asthma.1 Similar results were noted after treatment with oral prednisone.2, 12 Aerosolized budesonide therapy together with nebulized salbutamol early in the treatment of acute moderate exacerbations of asthma assisted in early recovery.13 It is uncertain whether a single dose of nebulized steroid could effectively decrease airway
