    A system that can automatically detect the EFL does not exist, which explains why studies about EFL are confined to research settings. Judicious use of PEEP reduces work of breathing in spontaneously breathing patients with dynamic hyperinflation by reducing the inspiratory threshold load generated by the difference between airway and alveolar pressure during invasive ventilation and NIV.15,19,36,58,78-80 The effect of PEEP differs according to the mechanism of hyperinflation.79

    Addition of external PEEP usually lowers the expiratory driving pressure. However, in patients presenting with expiratory flow limitation, external PEEP will not lower the expiratory driving pressure until the level of external PEEP exceeds the level of auto-PEEP by a ratio of 50 to 75%.24 26 27 External PEEP was variable in our study.
