La ventilation à l'Ambu® chez le nouveau-né : une simple question de doigté ?Is optimal Ambu® ventilation of the newborn a simple question of finger-touch?

Résumé

Les auteurs ont mesuré les pressions d'insufflation lors de la ventilation manuelle du ballon auto-insuffleur néonatal (Ambu®) d'un mannequin nouveau-né. Les pressions maximales obtenues (valcurs moyennes : 38 cmH2O) sont supérieures à celles rapportées dans la littérature et annoncées par le fabricant, théoriquement limitées à 30 cmH2O à valve ouverte, et ceci quel que soit le nombre de doigts utilisés lors de la compression du ballon. Ces résultats indiquent que la ventilation à l'Ambu®, souvent indispensable en salle de naissance, n'est pas une simple question de doigté et qu'elle comporte toujours un risque de baro-et/ou volotraumatisme.

Summary

Insuflation pressures were measured during manual ventilation using a neonatal rebreathing bag (Ambu®) on a manikin. Maximal insufflation pressures were greater than that published or given by the manufacturer, theoretically limited to 30 cm of water at open valve, and that wathever the number of fingers used for the compression of the bag. These results indicate that Ambu® ventilation, often mandatory for newborn resuscitation, does not simply rely upon the finger-touch of the operator and that it always has a risk of baro and/or volotraumatism.
