We have read the work of Chen et al1 and believe that it is an important contribution to the current scientific evidence. However, we believe that it is necessary to analyze some points. Nocturnal ventilatory support is a real challenge. On the one hand, the professional in charge becomes the “David” of this story, as we face the dilemma between moving toward liberation or minimizing interventions so as not to generate sleep disruption or agitation. Adequate programming of pressure support (PS), regardless of mode, is the “Goliath” of every story. In any case, there is no evidence to support the claim that pressure controlled ventilation (PCV) offers rest to the ventilatory muscles, in order to strengthen them, or that it increases the likelihood of successful weaning from mechanical ventilation.
Chen et al1 should be congratulated for their effort in bringing light to the topic, …
